The 2026 European Society of Cardiology (ESC) Guidelines on Heart Failure have introduced significant changes to the classification of heart failure (HF). Notably, the previous classification of HF with mildly reduced ejection fraction (HFmrEF) has been eliminated, streamlining the focus to two primary phenotypes: HF with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF). This shift aims to enhance clarity in diagnosis and treatment strategies.
Additionally, the guidelines have established new staging metrics for heart failure, categorizing patients into four distinct stages based on their risk and symptoms. The terminology has also been updated, replacing 'acute HF' with 'decompensated HF' to better describe the gradual decline in heart function. These changes reflect a growing recognition of the need for early intervention and tailored treatment approaches to manage heart failure effectively.
